This is a general thread for discussing where we purchase our equipment and tools. Why? Because everyone needs this sort of resource!
The main place I purchase stuff from is Adorama Camera. I find that often they have deals that beat out B+H, with the same great service that people know B+H for. They have a wide selection of items as well, and a great lab service. So Adorama is always the first place I turn to for photography equipment.
I have more sites, but I'll wait to hear in from everyone else before I spam you with more of my opinions. ;D

I actually don't have any particular place I look for things. I look on the internet for what I need, maybe a Google search, then spend a long time comparing products, reviews, etc. for the best one I can find quality and price-wise. xD Though if I found some good company I might always look to them first. =P

I just go to my local camera store.
I do get the catalog from B+H, but I hate to pay shipping costs and then I worry the whole time that the equipment will be damaged on the way over.

They package the equipment pretty well, so there's really not much to worry about damage wise. But yeah, shipping fees suck.

I go where ever, stay local and shop at San Jose Camera Video or go for a drive and visit the nice Keeble & Shuchats (B&H minus converys), Owens Originals, B&H, Amazon. Etc. I just go to my local camera store. I do get the catalog from B+H, but I hate to pay shipping costs and then I worry the whole time that the equipment will be damaged on the way over. I ordered photo paper and it came in a box four times the volume than the box of paper, haha.

It depends.
Over the years, I have spent A LOT of money with B&H. They have good prices, and great customer service. Lately though, I have been buying some stuff from Amazon, and have had good luck with them also.

I haven't had to buy any equipment since I received my digital camera as a gift 3-4 years ago. Well I picked it out, but Hubby said it was my Christmas present. If I remember correctly we bought it on sale from Circuit City. Oh, I do burn everything to CD/DVDs though, which I buy at Wal-Mart. Does that count as equipment? If I were ever to buy a more advanced camera though, I'd probably get it from a local camera store, Amazon or eBay. I've never heard of B&H or Adorama.

B&H and Adorama are certified camera dealers, and both have excellent prices.

Vistek and Henry's are the two major/comprehensive retailers in Ottawa. There's a Henry's location in my neck of the woods but lately I've been heading downtown and buying stuff from Vistek. I buy most of my film from them and saved about $200 on my 70-200 when I bought it last year. Henry's has a pretty atrocious website, too.
